New options for enteral nutrition in patients with severe acute pancreatitis




Severe acute pancreatitis (SAP) requires an adequate nutritional support. Enteral nutrition (EN) should be preferred to total parenteral nutrition in patients with SAP, as it is associated with reduced mortality and complications. However, in clinical practice EN is employed far less frequently than it should. The main obstacle to EN diffusion is that it is considered complicated, as to ensure full pancreatic rest, nutrition tubes should be placed in the jejunum, requiring often troublesome procedures. In the past few years, it has been proposed that EN through nasogastric (NG) tubes may be a simple, safe and equally valid alternative to nasojejunal tubes.


A research article published on August 7, 2010 in the World Journal of Gastroenterology addresses this question. The authors speculated that a pragmatic possibility in real-world clinical practice would be to employ NG feeding whenever tube migration to the jejunum of bedside inserted feeding tubes does not occur spontaneously. They therefore aimed at assessing the rate of spontaneous distal migration of EN tubes in patients with predicted SAP, to identify possible factors associated with it, and to compare the safety and tolerability of EN with an elemental formula in patients who started nutrition with a "proximal", NG or a "distal", naso-intestinal tube, depending on the success of spontaneous tube migration.


This is the first study of its kind observing the outcome of EN in SAP patients in a "real world" clinical setting, with the study protocol driven by the need to have more solid grounds in making clinical decisions about everyday medical care circumstances. Both the proximal and the distal enteral approaches resulted to be feasible, safe and effective in most patients. This issue has a relevant impact on everyday clinical practice as the main limit to EN usage in AP is the technical difficulty in obtaining small bowel access.

Weight-lifting is something I have been interested in all my life. I love the way I feel after a workout with weights, but most of all I love the way weight-lifting makes my body look. Have you ever thought you would like to get into weight-lifting but just did not have the time? Well, here are my thoughts on weight-lifting for the half-hearted.

Four years ago, I moved alone to a rather remote and isolated area where I knew absolutely no one. It was a tiny little town, and there was nothing to do there. I spent all my time writing and lifting weights. There was no gym in the area so I invested in several different sizes of dumbbells and a small, inexpensive weight bench. I did not spend much money at all as dumbbells do not cost much if you get the plain, non-pretty ones, and I think I paid about $25.00 for the bench.

I spent some time online looking up weight routines until I found something that worked for me. Each day, I worked an hour on either the upper body or lower body. Unfortunately, I did no cardio due to the odd circumstances of my living environment. In just a few short weeks I was in great shape with well-defined muscles. I loved how I looked and felt.

I maintained this physique for a couple of years, and then I moved to the city. My lifestyle changed completely, and I stopped weight-lifting. I am all soft now. I have been wanting to get back into my routine, but I currently work a full-time job, a part-time job and am a full-time student so I don't have a lot of time to devote to much else at the moment.

A few days ago, I remembered something that has helped me to pick up those dumbbells again. For awhile, I dated a high school track coach. I told him one day that I was into weight-lifting but had only had time to do a little bit, and I was disappointed that I could not do more that week. He grinned that crooked, lazy, boyish grin of his and said, "S'all good."

You know...it really is "S'all good." I realized that doing one set of anything with a pair of dumbbells IS good. It is better than not having done anything at all. With that thought in mind, I put my favorite pair of dumbbells next to my desk. Now, when I get up to get a glass of water or take a break from my work I see my dumbbells, pick them up and crank out a short set. After just a couple of days of doing that, I already see that my arms are tighter.

So...there it is, folks...weight-lifting for the half-hearted is good, and you can do it too.

Yankees What Happened To Our Don?







Joe Torre is undoubtedly one of the most successful managers in sports history. Starting as a manager for the Yankees in 1996, Torre went on to manage the team for 12 straight years, bringing them to the playoffs in each year. Also add in four championships.

At the end of the 2007 season, Joe Girardi took over as the Yankees manager, and Torre went on to manage the Dodgers. Three years later, he has announced that he will be stepping down as manager of the Dodgers in 2011, and will hand over the job to Don Mattingly.

As most Dodgers fans expected, Joe Torre is stepping down as manager of the Los Angeles Dodgers and hitting coach Don Mattingly will be replacing him.


I don’t know how well Mattingly will do as a manager because he has never managed at any level, but I don’t think it will really matter who the Dodgers' manager is.


Until the chaos of the divorce case between Frank McCourt and his wife, former Dodgers executive Jamie McCourt is resolved, the Dodgers will not be able to improve their lineup.


In today's world, you can build a good nucleus of a team with home grown talent, but if you expect to win a World Series, you have to be able to compete in the free agent market. The Dodgers will not be able to do that until they get new ownership.


For the most part, you can't buy a World Series by simply acquiring the best free agents on the market. But you do have to be able to pull the trigger if someone valuable to your team becomes available.


The Dodgers need at least two reliable starting pitchers and a good closer.  They are also in need of some hitters who can get the big hit with men on base, in addition to someone who can hit the long ball on a regular basis.


There are also rumors that Andre Ethier, Matt Kemp and some of the other Dodgers want to play elsewhere. Can you blame them?


With the whole divorce situation between the McCourts, the Dodgers are fast becoming the Oakland Raiders of Major League Baseball.

Top Reasons The Middle Class Is Disappearing

Where have you gone middle class American family? Are you hiding in the gas tank of your new hybrid? Are you buried under the wood that sits and waits for the first snow flake to fall. Or are you in the fridge, next to the five dollar gallon of milk? My answer is, all of the above.

I was once firmly ensconced in the comfy of middle class America. I slept very well back then. I ate well too. I only took aspirin when I had a cold. Now, I seldom sleep, eat mounds of inexpensive junk foods and take my Motrin just to get through the stress of the day. The sad thing is, there are millions just like me out there who have been shifted down not one but two financial classes during the last 8 years. What was Upper/Middle is barely holding on to Lower/Middle these days.Those who were Upper/Lower are now staring poverty straight in the face. Was this simply the result of an inept president or was it a more than that. Was this the first truly "perfect storm" of financial proportions since the Great Depression. Sure there have been other rough times but seldom have the ugly fingers of economic despair reached so far, so deep and so griping. I know consider myself Upper/Lower class.

My wife and I both have well paying jobs, her in administration and I in Radio advertising yet we have been brutalized by the power of this omni-present black
cloud called the American Economy. What many people fail to realize is that the double edged sword of inflation and a bad market place is the fact that wages are often stagnant while prices continue to climb. Businesses can't afford to offer there employees higher pay for the simple fact that they have less customers. This is especially prevalent in sales based professions.

I was recently asked by my manager what I was planning on doing to counteract this economical spiral. I looked him in the eye and told him that if I had the answer to that question my name would be on the ballot in November.

Where have you gone middle class. We sure do miss you.

Whether you’re a weekend warrior or an all-out athlete, sports injuries can occur to anyone at any given time. The good news is, sports injuries usually can be treated fairly quickly and with simple procedures. Even better, many of these injuries can be avoided with a bit of education as to proper preventative procedures. 

According to the National Institute of Health, the most common types of sports injuries are: 

· Muscle sprains/strains

· Tears of ligaments holding joints together 

· Tears of tendons that support joints 

· Dislocated joints 

· Fractured bones, including vertebrae

Each sport has its own set of injuries unique to the physical stresses and demands place upon your body. Before you hit the road, court, or any other surface, it’s good to know what you’re up against and how to safely meet all the challenges involved.

Basketball 

A quick game of pick-up basketball is great for spirits as well as for the cardiovascular system! But it’s also responsible for some 1.6 million injuries each year. The American Academy of Orthopaedic Surgeons offers these tips to keep you safe on the court: 

· Warm up first! Jumping jacks, stretching and even walking in place is a good way to get those muscles ready for the game. 

· Wear shoes that can do the job. Even though your favorite sneaks feel good on your feet, they may not be up to the task. Look for shoes with ankle support to reduce sprains, and make sure the shoes fit snugly and have non-skid soles. 

· Check the court before you begin. Small rocks, holes or debris can turn a flat surface into a playing hazard.

Bicycles 

Biking is a popular form of exercise that is suitable for all ages. Unlike many sports, it doesn’t require a great deal of skill to generate a substantial amount of enjoyment. According to the Association of Family Physicians, the group with the greatest number of bicycle-related injuries and fatalities is males, age 9 to 15; accidents are more likely to occur in the summer months. 

That doesn’t mean you should keep your adolescent son off the bicycle; instead, make sure that he – and everyone else in the house who gets on a bike – follows these safety measures: 

· Wear protective gear! Helmets reduce the risk for head and face injuries by as much as 85 percent. Make sure the helmet is worn correctly – snug around the head and in a horizontal position. Don’t stop with the helmet – gloves and eye protection are important as well. 

· Watch for cars. Accidents involving motor vehicles are the second leading cause of injuries and deaths on bicycles. In fact, off-road cyclists have a 40 percent lower rate of head, facial and dental injuries, thanks to the fact that they aren’t near cars. Children under the age of 10 should not be allowed to ride in areas where cars travel. 

· When cycling off-road, know the area. Take it slow on unfamiliar paths, where loose gravel or sharp turns can produce some unexpected surprises. And always perform basic maintenance on your bike before taking it out for a ride.

Golf 

Because golf requires a relatively low level of physical activity, most people don’t consider that injuries can occur. But hitting the links can be hard on the elbow, spine, knees and hips if you’re not prepared. To warm up, doctors advise: 

· Squeeze a tennis ball for about five minutes at a time; it will strengthen your forearms and help avoid elbow injuries. 

· Build those back muscles! The lower back is susceptible to injuries, and the best way to prevent problems is through regular exercise. Rowing and pull-down exercises are ideal ways to work the back muscles. 

· Reach for a good game. Stretching out before you play is one of the most simple – and most effective – ways to get your body ready for a few holes. If you’re riding in a cart, don’t forget to stretch after sitting and before swinging.

Inline skating 

Current statistics indicate that some 26 million Americans participate in the sport of inline skating. That leaves a lot of opportunities for emergency room visits! Skaters who exercise prevention will find themselves ahead of the pack in warding off injuries. Some of the best ways to do that are: 

· Choose a boot that fits properly. This means selecting your boot at the end of the day, when your feet are largest, and also wearing the same type of sock you’ll wear when skating. Make sure your feet are all the way inside the boot – with heels touching the back of the boot – before buckling up. (If your heel moves up and down while skating, it’s not the proper fit.) 

· Get your gear on. A helmet is a must-wear, but it doesn’t end there. Elbow and knee pads, as well as wrist protectors, are as essential to inline skating as the skates themselves. 

· Warm up. See a recurring theme here? Stretch your muscles and do some warm-up exercises for your legs before hitting skating. When you’re done, stretch out again, just as you would after a jog in the park.

Tennis

Tennis is considered one of the safest sports around, but it can also lead to injuries without proper preparation. One of the most common injuries is tennis elbow, an inflammation that results from either a poor backhand or gripping a racket that is too small. Pulled muscles are another problem for tennis players, but injuries can be avoided if you: 

· Stretch and warm up before games. Spend 15 to 30 minutes before your game stretching out and warming up; it will improve your flexibility, circulation and could even improve your game. Don’t forget to jog in place to get your blood flowing and get your heart ready for activity. 

· Practice proper form and fitness. This means not just learning how to handle your racket, but caring for your muscles off the court as well. Strong muscles are less susceptible to injury, so a regular fitness routine can minimize sprains, strains and tears.
